Work-Based Learning Experience

Dear Students,

We are delighted that you have decided to register for a Work-Based Learning (WBL) experience. The WBL team is here to offer you support throughout your experience.

In the next few pages, you’ll be asked to complete the required application materials. It is important that you complete the forms and hit Submit for the Work-Based Learning staff to approve.

Special Note:

You will be registered for WBL when:

1. Your approved employment has been verified by the Faculty Coordinator and/or the Work-Based Learning staff, and
2. All required WBL documents have been received by the Work-Based Learning staff.

Accident Insurance Notice

FTCC students participating in a Work-Based Learning (WBL) course are covered by a $10,000 accident insurance policy. However, some students may be required to obtain additional professional liability insurance through Fayetteville Technical Community College.

Please note: FTCC is not liable for any accidents or injuries that occur during your WBL employment. Also, WBL enrollment does not qualify students for unemployment compensation upon completing the program.

Statement of Understanding

By signing this application, I authorize Fayetteville Technical Community College to disclose all information on this form to my Work-Based Learning employer or any potential employer.

I understand that:

• I cannot start counting work hours until the first day of the semester.
• Work-Based Learning is a graded academic course that may earn academic credit.
• I must work the required number of hours as stated in the course description.
• I must maintain accurate timesheets signed by both my employer and myself.
• FTCC does not guarantee permanent job placement.
• I agree to follow all Work-Based Learning policies and submit all required paperwork on time.
