The following forms are offered for your convenience in two of the most popular word processor file formats (Microsoft Word Version 6, & Word Perfect Version 6).
Just click on the file format for the word processor you have access to and select "save to disk" to download the file to your disk. If your Web Browser has "Active X Control" installed, it may directly open the word processor if it is installed on the computer. In this case, just print while the program is open, or do a "save as" to save the file to your disk for future uses.
The MS Word forms are easily filled by typing directly into each box then tabbing to the next. The WordPerfect forms can be typed on, however, you should use the mouse to position the curser in each box then type the required information.
