Student Participation Confirmation Form
Student: Completing this form will confirm your participation in an online, hybrid or telecourse/CBC class. The College is required to provide documentation that you are actively participating in your class between the first day of class and the 10% point. Please enter the information below and click on the 'submit' button. If you are taking more than one online class, you will need to complete this requirement for each class.
