Vision benefits are provided by Superior Vision.  Choose Plan #1 for full benefits (eye exam, eyeglasses or contacts) or Plan # 2, a materials only plan (eyeglasses or contact lenses). There is a $20 co-payment for exam only.   In order to maximize benefits please visit the attached website to locate an in-network provider.  Replacement cards may also be requested at the attached site.
